Induction of Labour in a Hospital setting – Part Two

There are various methods of IOL in Australia and depending on which hospital you attend, you may have different options available. We will explore some of these methods as well as discuss some of the associated risks that come with being induced.

Firstly, your midwife/obstetrician will usually perform a vaginal examination to ascertain a ‘Bishops score’. This is a system which determines how ‘ripe’ your cervix is and whether there have been any cervical changes.

Prostaglandins

  • – Usually comes in the form of a gel or a flat, tampon-like structure that inserts into the vagina, behind the posterior fornix of the cervix.
  • – Releases Dinoprostone, a prostaglandin which is meant to mimic the body’s natural process of softening and dilatation.
  • – A CTG will be applied before and after for a time, to ensure that your baby is ok with this change in the body.

Immediate Risks & things to consider:

  • – Vaginal discomfort
  • – Potentially, small amounts of bleeding during insertion from irritation
  • – Rarely, the uterus will contract too many times and then you will be given another medication to relax the uterus
  • – You will usually need further intervention to help you get into labour.

Balloon Catheter

  • – A catheter with two balloons are inserted and filled with 60-80mls of water each. One balloon goes inside the cervix and the other goes in the vagina, parallel to it.
  • – The aim of is to cause pressure and send your body signals to start contracting.

Immediate Risks & things to consider:

  • – Can cause a lot of pressure and can be quite uncomfortable. The midwife/obstetrician may take some of the water out if it is too severe.
  • – Left in for at least 12 hours, and then taken out.
  • – Will usually need further intervention to get the labour going
  • – No artificial hormones used with this option
  • – You can move freely with this in. Some hospitals let you go home with it in overnight.
